Takeaways from the Emory Aesthetic Center’s Skin Care Live Chat

During this live chat, Dr. Anderson and Danette discussed treatments and ingredients to keep your skin at its optimal health. They provided general skincare guidelines for every stage of life as well as cosmetic treatment options to reduce the signs of aging and keep you looking younger. Question: What can I do to reduce facial dark spots?

Dr. Anderson: Dyschromia and hyperpigmentation occur as our skin ages or with hormonal changes like pregnancy. There are many approaches to address the condition depending on your skin type. Hydroquinone therapy is the gold standard of treatment and is applied daily to the skin. There are also non-hydroquinone based treatments available that work similarly. Many light-based treatments exist to remove or lighten these lesions as well, such as Intense Pulsed Laser (IPL) or Fraxel laser.

 

Question: What’s the best non-invasive way to treat smile lines?

Dr. Anderson: The main way to treat lines around the mouth is with fillers. It is minimally invasive, meaning that the product is injected under the skin as an office based procedure. A regimented skin care program with chemical peels on a monthly or every 6 weeks basis will also improve wrinkles and lines on the whole face.

 

Question: I am 57 and don’t really have any wrinkles or problems and would like to keep it that way! Do you have recommendations for a skin care regimen? I have normal skin.

Danette: Using antioxidants (vitamin C, green tea, or products you can purchase in a medical spa that have active ingredients to promote skin health) every morning, applying sunscreen! sunscreen! sunscreen! daily, and using a light-grade retinol cream at night can help protect and maintain your skin. You should also see a licensed aesthetician at a medical grade spa once a month to ensure your skin stays looking its best.

Takeaways from Dr. Eaves’ Body Contouring – Getting Ready for Summer Live Chat

Body ContouringThank you to everyone who joined us for the Body Contouring – Getting Ready for Summer live chat hosted by Emory Aesthetic Center surgeon, Dr. Felmont Eaves. Dr. Eaves discussed body contouring options to help shape the body for summer and tighten excess or loose skin. He described both surgical and non-surgical procedures – including liposuction, tummy tucks, arm and thigh lifts.

We were thrilled with the number of people who registered and were able to participate in the chat. The response was so great that we had a few questions we were not able to answer so we will answer them below for your reference.

Question: What are new procedures for arm lifts?

Felmont Eaves, MDDr. Eaves: Arm lifts continue to evolve as we are doing more of them all the time. Arm lifts can be categorized into short scar techniques, full length techniques, and minimally invasive techniques. The short scar technique leaves a scar that is limited to the axilla (arm pit) and can help looseness in the upper part of the arm closest to the shoulder. But this technique isn’t as effective if the looseness goes far down the arm. In this case a full-length arm lift is recommended, although this does leave a long scar on the inside of the arm. For some patients, if skin laxity of the arms isn’t severe, liposuction alone may give a very nice result with a tiny scar. While there are some companies advertising significant arm tightening with non-invasive procedures, I haven’t seen impressive results with these yet.

Question: I have a normal BMI. How drastic can I expect the results of CoolSculpting to be?

Felmont Eaves, MDDr. Eaves: CoolSculping can certainly make some nice changes, but the results tend to be more subtle than that achieved by liposuction (or excisional surgery). However in patients that have excellent skin elasticity and limited fat deposits, it can do a great job of addressing what you want. It really takes an examination by someone who is familiar with all types of body contouring techniques (non-invasive, liposuction, and excisional surgery) to help you figure out which is best for you.

Question: I have a scar on my stomach from a childhood surgery. Can you fix this?

Felmont Eaves, MDDr. Eaves: That’s a good question. If you are talking about making a scar looking better, then yes, a scar revision can help. Even further, if you have a scar in the lower part of your stomach (below your belly button), that scar might be totally removed with a tummy tuck (although you’ll get a new tummy tuck scar – but it will be better positioned and hidden within the panty or bathing suit area).

Question: Can you get a 6-pack look with a tummy tuck?

Felmont Eaves, MDDr. Eaves: It is possible, but not usually. Having a real “6-pack” requires two things – well developed abdominal muscles (specifically the two muscles in the middle, known as the rectus abdominus muscles) and a very low body fat percentage. When we perform a tummy tuck, we thin out the fatty layers of the abdomen, however it is important to keep the thickness of the tissues in harmony with surrounding areas of the body, otherwise it may look odd. A tummy tuck won’t build up the abdominal muscles.

Is their a technique such as skin tightening that can decrease hollowness around the eyes and decrease the appearance of dark circles?

Foad Nahai, MDDr. Nahai: Sometimes, shadows resulting from contour irregularities, or hollowness, around the eyes can accentuate the dark circles. Fillers may be a solution in these situations. Also, skin care including retinols, skin bleaches, as well as surgical intervention, such as an eyelid lift, may help to improve this.

Is there a difference in the outcome or a reason to use microdermabrasion versus use of a laser on the face?

Foad Nahai, MDDr. Nahai: There is a very significant difference between the two. Microdermabrasion is a lighter therapy and is used for more superficial purposes. Laser resurfacing, however, can range from light and superficial, to heavy and deep. Both are effective for minor skin changes but only lasers can be used for more significant or deeper problems.

How can I get rid of under the eye wrinkles and is there something that can be done to whiten eye sclera?

Foad Nahai, MDDr. Nahai: There are several options for improving eye wrinkles that include creams, facial peels and surgical skin removal or lifting. After examining your skin, a specialist will be able to recommend the best treatment for you. There are eye drops available which can help to improve “blood shot” eyes, but there is not a procedure to whiten the sclera.

I fell a couple years ago, and my forehead hit a curb right above my eye. Of course I had a huge black eye as a result. It has been 2 years and I still have darkness around that eye – a shadow in the same shape as the black eye. Can anything be done to lighten this?

Foad Nahai, MDDr. Nahai: Dark circles under the eyes represent one of the most challenging areas for improvement. The eyelid skin is extremely thin, with minimal if any fat between the skin and muscle. However, there are non- surgical options, such as retinols and skin bleaches, and surgical options that can help provide some improvement.

Question: Is there anything you recommend for the treatment of rosacea?

Foad Nahai, MDDr. Nahai: A variety of topical agents are available to treat rosacea. After an evaluation of your condition by a dermatologist, he/she should be able to advise you on the best treatment options for you.

Weight Loss ChatThank you for attending the live chat on Tips for Losing Weight and Post Weight Loss Body Contouring on Tuesday, July 22nd. We had a great discussion, so thank you to all who participated and asked questions. We were thrilled with the number of people who were able to register and participate in the chat. The response was so great that we had a few questions we were not able to answer during the chat so we will answer them below for your reference.

How expensive is a pannus roughly?

Felmont Eaves, MDDr. Eaves:
That depends on the size of the pannus and how far it extends toward the side. Please feel free to call the Emory Aesthetic Center for some general guidelines, and we’d be happy to see you and figure out the extent of surgery required and costs.

 

How do you feel about the “cryo” procedures to the neck area? Are they effective if one is 60 or older? Will the procedure prevent or tighten the turkey neck?

Felmont Eaves, MDDr. Eaves:
I suspect that in patients older than 60 years of age that non-invasive procedures to remove fat in the neck area will have limited utility due to the looseness of the skin, and are unlikely to have significant impact in tightening a turkey neck. A neck lift, however, can create exciting results.

 

Is it possible to achieve biggest looser weight loss numbers through training at home?

Arvinpal Singh, MDDr. Singh:
The Biggest Loser and other television shows that show extreme weight loss are unrealistic for most of us who are working full time and have other time commitments. Those patients are seeing nutritionists, physicians, and physical trainers on a daily basis, if not more. For any weight loss program, consistent cardiovascular and strength building training is appropriate, combined with a healthy eating plan. Realistic weight loss goals should be one to two pounds per week.

That said, everyone can absolutely eventually achieve their ideal body weight, regardless of where they are starting off. The most important thing is to really clean up your diet – centering it around these four major food groups: vegetables/greens, fruits, whole grains, legumes/beans/lentils. Meat and dairy should be treated as side dishes or condiments. Oils should be minimized. Healthy fats from nuts, seeds and avocados can be used, but in small amounts while you are trying to lose weight. Exercise also is very important because it has significant cardiovascular benefits, and is very important at maintaining the weight loss long term.

My insurance excludes bariatric surgery, do appeals ever work?

Arvinpal Singh, MDDr. Singh:
Rarely. Insurance exclusions are often chosen by the employer. If bariatric surgery is excluded, it is very unlikely that you will get approved for surgery unless the employer changes the coverage. You may want to try to talk to your HR department. Explain your situation and discuss how bariatric surgery will help to improve your medical issues.

 

Which exercise works with getting mid section in shape?

Arvinpal Singh, MDDr. Singh:
Everyone wants a slimmer waist line! All core/back exercises are great at strengthening the core (yoga, pilates, planks, etc). Cardio exercises (brisk walking, jogging, swimming, rowing, etc.) to get the heart rate up are also helpful with burning fat and reducing weight. The most effective way to reduce inches off the waistline is to clean up your diet, especially reducing the saturated/trans fats, added oils, and refined sugar. Abs are not made in the gym, they are made in the kitchen!
